If you are entitled to health care under EEA rules on coordination of social security systems during a temporary stay in Poland you are entitled to receive free health care services, necessary on medical grounds, from health care providers who have concluded contracts for health services with the National Health Fund (NFZ).
It is our pleasure to inform that the Carolina Medical Center signed an agreement with the National Heath Fund (NFZ) for medical services provided to the youngest patients.
Bearing in mind that orthopaedic treatment of children differs substantially from the treatment of adults, and most surgeons do not specialize in treating young patients, the Carolina Medical Center decided to grant access to its doctors’ experience in treating children as well as modern equipment to all youngsters requiring orthopedic surgeon’s assistance.
The contract package includes:
1. Hospital care
Paediatric surgery and Trauma-orthopaedic surgery departments
2. Specialist outpatient
Trauma-orthopaedic surgery clinic for children
Paediatric surgery clinic
Sports Medicine Clinic
Magnetic Resonance Laboratory (MRI)
Computed Tomography Laboratory (CAT)
3. Physiotherapy
Physiotherapy Clinic for children
Musculoskeletal Clinic for children
Documents needed to be provided in order to benefit from the service:
· in case of short visit in Poland (tourist) – passport and European Health Insurance Card EHIC (or a replacement certificate)
· in case of living in Poland – passport, parent’s insurance confirmation from the employer and child’s insurance confirmation (form ZUS ZCNA or ZUS ZCZA)
